Uplifted Care - Hospice of Kankakee Valley
Provides compassionate physical, emotional, spiritual, and social support for individuals living with terminal illness and their families. Also hosts a variety of grief support groups that are open to the community. Medicare and Medicaid recipients are never billed for hospice services, and the cost for patients who do not qualify for Medicare, Medicaid or insurance, or who owe co-pays or deductibles, is determined on a sliding scale.

Services

Grief Counseling

Individual counseling is available in-person and over the phone. A family grief camp is held annually in late summer. All services for grief support are free and available to the whole community whether your loved one was receiving services from Uplifted Care or not.

Grief Support Groups

Bereavement Support Groups are open to anyone in the community who has experienced the death of a loved one within the last year. Separate groups are available for adults, children, and teenagers. A series of different specialized groups are offered throughout the year. Check Uplifted's website calendar page for dates and times, or call (815) 939-4141 and ask to speak with a bereavement counselor. The ongoing adult support group provided all year long are: 1.) 1st Tuesday of every month at HKV at 10:00am located in the counseling room. 2.) 1st and 3rd Thursday of every month at HKV at 6:00pm located in the counseling room. All services for grief support are free and available to the whole community whether your loved one was receiving services from Uplifted Care or not.

Hospice Care

Provides: Comfort Care and symptom management, support and education for the family, medical supplies and equipment, counseling for the patient and family, spiritual support, regular nursing visits, 24-hour on-call nursing support, volunteer assistance, medications related to the terminal illness, grief support for the family. **Nursing services available 24/7 by calling 815-939-4141.

Palliative Care

Palliative care is specialized medical care for persons with serious illness who require an extra layer of support while still receiving curative treatment. Visits are made by physicians and nurse practitioners, social work and spiritual care support to assist with symptom management, coordinate community resources and assist with advanced care planning.

Transitions Program

This is a free, non-medical program that provides volunteer support and resource management to persons with serious illness who may still be seeking curative treatment. Weekly assistance with things such as companionship, light household help, meal prep and respite for caregivers is available.
