Community Health Partnership of Illinois (CHP) is a Federally Qualified Health Center (FQHC) whose mission is to build and deliver quality, culturally responsive healthcare to our patients, caregivers and the communities we serve. CHP empowers individuals, including the uninsured, underserved, migrants & seasonal agricultural workers, to attain their best health and well-being. Our network is comprised of 6 health & dental clinics and multiple outreach sites across northern and central Illnois.

Services

Dental Services

- Basic dental care services including: cleanings, exams, fillings, extractions and anterior root canals (front teeth)- Periodontal (gum) treatment- Emergency dental care- Pediatric dentist visits for very young children- Oral health education

Medical Care

- Primary care services for all ages- Well-child care (regular checkups, school physicals)- Lab and basic diagnostic services- Screening for diabetes, hypertension, colon cancer, cervical cancer and other health conditions- Chronic disease management and support for patients with diabetes, hypertension, asthma and other chronic conditions- Womens health services, including breast cancer screening and Pap smears- Care coordination

Telemedicine Support Services

- Assistance paying for prescription medicines- Bilingual (Spanish-English) interpretation, care coordination, and health education- Help applying for public benefits including All Kids, Medicaid, Obamacare, SNAP benefits and other programs
