Care & Support Services

Hospice provides care through a team of interdisciplinary caregivers consisting of physicians, nurses, home health aides, social workers, chaplains and volunteers. This team works together with the patient and his or her family to create a comprehensive plan of care. Hospice provides: visits by nurses, management of pain, short-term inpatient care, personal care, coordinated services with patient's physician, help in obtaining financial assistance and other community services, spiritual and emotional support, supportive care by trained volunteers, family counseling, respite care, therapies and nutritional consultation, and bereavement support for family members.
