Early Intervention Services

Individual Service Coordination

A program of Service, Inc. of Illinois providing services for children age 3 -17 and adults 18 and older with a developmental disability. Serves Will, Grundy, and Kankakee Counties. Individual Service Coordinators (ISC) assist individuals and families in finding and maintaining developmental disability services. ISCs provide case management and service coordination, which includes enrollment in the state's waiting list (PUNS), determining clinical eligibility, assisting with identifying providers of choice, assistance with finding funding sources, developing an individual's Personal Plan, and monitoring their health, safety, and well-being. ISCs serve as the main resources for information, support, and advocacy to help individuals and families make informed choices and navigate the developmental disability system.
